We can make the necessary phone calls, make connections to appropriate agencies and services, and help with paperwork, manage caregivers, and be your guide through the senior healthcare world.

Choice Care Navigators will do the research to locate and vet providers to specifically meet your needs. Want to know which home care agency fits your needs and how much it costs? Looking to move to an Assisted Living Community close to your house? Need an elder law attorney? Tell us what you are looking for, and we will help you find it.

Ignite! is a customized, personalized activity program that enhances quality of life. Each client’s program is based on their personal interests, strengths, and limitations. Services are available to individuals with all diagnoses and activity levels, wherever they call home.

What will they do? Programming ranges from in-home visits to outings of the client’s choosing. Painting, creating a memory book, playing Scrabble, singing together, latte’s at the park…these are just a few of the possible enrichment activities. Each session is tailored to the individual’s wishes, making it fun, purposeful, and engaging.

Who should participate? Anyone who is experiencing isolation or loneliness, depression, anxiety, or cognitive decline, or anyone who would like to have more to look forward to during the day!

What are the benefits? Reduce isolation, improve quality of life, and engage your loved one socially, physically, emotionally, and cognitively.

Guardianship is a legal intervention utilized when an individual can no longer make important life decisions. This may include decisions about his/her person (health decisions, living arrangements, etc.), property, or that the individual has become susceptible to fraud or undue influence.

In these situations, a Guardian can step in to care for them and make decisions on their behalf. Although family members and friends commonly serve as Guardians, this is not always possible. In cases where there is no one to serve as a Guardian, a professional may be recommended, such as Choice Care Navigators.

As Guardian, Choice Care Navigators serves as an advocate for our clients. We ensure that our clients have a stable and supportive lifestyle within the environment that works best for them. We want them to be safe and as independent as possible.

Additionally, if a friend or family member is serving as a Guardian, our Care Management Services can support them in fulfilling their role. Choice Care Navigators adheres to and follows the guidelines of the National Guardianship Association.
